You have to fill up the Passport Application Form and apply for "Re-issue of Passport". Fresh police verification is intiated if the passport has expired more than three years ago. Hence, fresh police verification will be required in your case.

In other words, passports issued to applicants age 16 or older can be renewed by mail up to 5 years after the expiration date (as long as the passport is not significantly damaged). Login to the Passport Seva Online Portal with the registered Login Id. Click "Apply for Fresh Passport/Re-issue of Passport" link. Fill in the required details in the form and submit. Click the "Pay and Schedule Appointment" link on the "View Saved/Submitted Applications" screen to schedule appointment.
