Passport annexure NOC:

No Objection Certificate (NOC) is issued in the format of  'Annexure-M' by the Controlling / Administrative Authority of the employee working under them for obtaining Passport by any Government Servants, PSU, Autonomous body employees, et al.

Declaration by applicant genetic parents for issue of passport to the child born through surrogacy. (On plain paper)


How can I get NOC for passport?

NOC on plain with the signature or stamp will be accepted. Prior Intimation is submitted by the applicant and should be submitted on plain paper. On all three documents, the telephone, fax and email ID of the controlling authority should be indicated so that the passport office can confirm the details.
